To prepare for this Assignment:
· Select an adult or older adult client with a bipolar disorder that you have seen in your practicum.
In 3-4 pages, write a treatment plan for your client. In which you do the following:
· Describe the HPI and clinical impression for the client.
· Recommend psychopharmacologic treatments and describe specific and therapeutic end points for your psychopharmacologic agent. (This should relate to HPI and clinical impression.).
· Recommend psychotherapy choices (individual, family, and group) and specific therapeutic endpoints for your choices.
· Identify medical management needs, including primary care needs, specific to this client.
· Identify community support resources (housing, socioeconomic needs, etc.) and community agencies that are available to assist the client.
· Recommend a plan for follow-up intensity and frequency and collaboration with other providers.
THE PATIENT
The selected patient is Jeff a Caucasian male 53 years old. Je ff was diagnosed with Bipolar II disorder 10 years ago. Jeff has been arrested in the past for theft of a car, theft of gas, and assaulting a police officer. He was released under court order to abstain from alcohol and attend AA meetings. For the past few months Jeff had been in a hypomanic mood. Since last week Jeff is now in the pits of despair. Jeff has never believed in medication except when forced from hospitalization. He came to the clinic with his sister. Jeff says he is open to a medication try and will attend therapy as necessary. Jeff is now seeking care because his sister who supports him financially said she will no longer pay his bills if he does not comply with treatment. Jeff lives alone with a dog. He does not drive or have a job.
