Select five individuals age >64 years of age
from your clinical rotation. Review their EMR and/or query the person to update
the EMR for CDC Recommended Immunizations appropriate for the age of the
individual. For each missing immunization, please provide a rationale for why
the immunization was not performed and whether that was appropriate. Indicate
if the individual should be encouraged to now get the immunization. You may use
a grid format. Please maintain HIPAA compliant documentation in your grid or
paper and upload it as a Word document.
·       Please use the CDC Immunization Schedule by Age CDC
Immunization Schedule by Age as your guide.
·       Ensure that immunizations have a date listed (MM/YYYY such as
09/2021)
·       For each patient, list the following demographics: Initial, age,
race, gender and residential status (such as resides at home; lives in group
home; resides at shelter; homeless)
·       You must include complete patient education about the disease
being discussed and the immunization delivery (allergies/live virus which
relates to comorbidities/after care) education. Simply stating that “All
patients are highly recommended to take (a particular vaccine) is insufficient
education about the disease itself or the immunization sequelae.
·       While you may use a grid to list your patients immunization
status and recommended vaccines, I would suggest a separate paragraph about
education for each disease/immunization.
·       You need 5 evidence-based scholarly references published
within the last 5 years to earn full points.
