Submit the completed IRB to your Instructor (after your TA has reviewed it) in one complete Microsoft Word document.

o Survey Questions: the whole Consent to Participate Voluntarily Form at the beginning of the survey and provide link to Google Survey. Make sure link is live.

? Carefully review your IRB before submitting. Strict compliance to following items are necessary to avoid delaying your approval process. Review the IRB sample in the Handbook Appendix.
**The CP-TA agreements [ON ORGANIZATIONAL LETTERHEAD WITH CONTACT INFORMATION] must be included as attachments to the Project Overview Submission Form in the IRB package.
1 Ensure the title of your research project is clearly stated and remains the same throughout your paperwork
2 In the Project Narrative identify how many people will be contacted and how many you hope to interview
3 If you plan to use, indicate how long the audiotape from the interview will remain it is recommended that the interview audiotape be deleted after transcriiption (usually it is kept for a period of 7 years in a locked safe in case any issues arise)
4 Review the Project Narratives Methods section, specify how the information will be kept confidential (state that any reports will contain pseudonyms and no names will be used; interviews will take place in a private but public area not in the persons home) where the participants responses will not be heard by anyone else; Consent forms will be kept in a locked cabinet)
5- Consent to Participate Voluntarily form must contain a sentence indicating that there are no potential risks to subjects participating in this research study
6- Consent to Participate Voluntarily form also must contain a sentence: In the event of a research-related injury to the subject or questions about rights as a participant, contact Springfield College IRB at 413-748-3959
7- Flyer How will you contact participants? (after they have responded to a flyer) The content of the flyer must include a scriipt, a structured set of statements that include your name, your affiliation to the school, and your purpose of the research. How will you obtain their name, how and where they can meet you for the interview? Revise the wording of the template to fit your project.
8- Your interview questions must be aligned with your research topic and scope
9- Your interview questions should be described in the Project Narratives Method section as semi-structured in that they are open-ended questions that will allow the participants to express their opinions on the subject matter and not just a yes or no answer. Any yes or no questions should be reworded to be open-ended.
10 The survey questions should retain the same intent as the interview questions so when you compare the two you will be able see the relationship in the answers given. A survey means taking the questions and reformatting them so people will have multiple choice type responses using words such as: select, rate, choose (yes or no), prioritize, etc.
