One example from my experience is that I had a chance to talk to a patient who was on so many different opoid for pain control. and patient had disease that current amount of opoid would not control his pain but his doctors did not bother to make a change. My pharmacist listened not only his pain concern but also his life concern to make him more confortable and show willingness to help.
