Create an evidence-based plan for the implementation and evaluation of care specifically designed for this vulnerable population
Be sure the plan is specific (i.e., if use of available resources is part of the plan, be sure to list the resources by name)
Provide a summary of the relationship between your learning and the Healthy People 2020 objectives
